Actor Matthew Fox has been formally charged in connection with his driving drunk arrest last week, RadarOnline.com is reporting.

The Lost star was charged Wednesday with two misdemeanors, driving under the influence and driving without a license, according to authorities.

Fox , 45, was taken into custody by police in Bend, Oregon, at 3:23 a.m. on May 1 on suspicion of driving under the influence while heading to a fast food restaurant for a late night snack. He was originally pulled over by police for failing to drive within a lane and failing to use his turn signal.

Matthew Fox is being sued for allegedly punching a woman last month, and RadarOnline.com has the court documents.

The lawsuit, filed in Ohio, claims that the Lost actor assaulted Heather Bormann after he tried to get on the party bus she was driving on August 28.

The Cleveland, Ohio woman who claims Matthew Fox repeatedly punched her last week has filed an assault complaint against the Lost star and prosecutors must now decide whether they intend to press charges against the actor and prosecutors will ultimately decide whether to press charges against the actor.

Regardless, Heather Bormann, 29, says she's going to file a civil suit against Fox.

"He hit me and he should pay the consequences of assaulting a woman,"  Bormann told Fox 8 Cleveland after filling out a statement about the August 28 incident at the Cleveland City Prosecutor's Office Tuesday. "Right now, it's considered a civil matter and that's why he was ultimately released Sunday morning, so I would have to press charges.”

Matthew Fox is accused of allegedly assaulting a 29-year-old woman in Cleveland, Ohio over the weekend, and RadarOnline.com has all the details.

Cleveland Division of Police Public Information Officer, Melissa Dawson, confirmed it was the Lost actor that was involved in the altercation, which occurred in the early morning hours of August 28, and was reported at 1:31 a.m.

“Victim states named suspect tried to board a private party bus that she was driving,” the offense report states. “Victim got up from the drivers seat and blocked the entrance.

Even before getting hot in Cleveland over the weekend, actor Matthew Fox has been at the wrong end of a pointed finger in the past. 

Last year, Stefani Talbott, a 27-year-old exotic dancer from Bend, Oregon, told the National Enquirer she'd slept with the 45-year-old Lost star. (Fox has been married to wife Margherita Ronchi for 20 years; the couple has two children, ages 9 and 12.)

"Yes, I've been having an affair with Matthew Fox,” Talbott, who met Fox while dancing at the Stars Cabaret, told the Enquirer in it’s March, 2010 issue. “We had sex together. I’ve got to be careful what I say — he’s a married man with two kids. But I kept voice mails and text messages from him.”
